Note

Base game location guide. This guide covers Sage’s Cave itself, including its two boss arenas and the connected cave passages. It does not include nearby Altus Plateau locations such as the Abandoned Coffin, Erdtree-Gazing Hill, Perfumer’s Grotto or Sainted Hero’s Grave.

Overview

Sage’s Cave is an optional cave dungeon in the western Altus Plateau. It lies beside a shallow lake and is unusually rewarding for its size, containing a large collection of equipment, consumables and crafting materials, as well as two separate boss encounters. The first is a concealed Black Knife Assassin, while the second is Necromancer Garris, an optional boss hidden behind an illusory wall.

The cave’s distinctive features are its concealed walls, skeleton enemies and large number of chests. Its Black Knife Assassin encounter is also one of the game’s early demonstrations of an invisible boss, while Garris introduces a necromancer supported by summoned skeletal creatures.

Location and boundaries

Sage’s Cave is a self-contained cave dungeon in the western part of the Altus Plateau. Its entrance is built into the edge of a shallow lake. The cave should not be confused with Perfumer’s Grotto, another cave-like dungeon in the same broad area, or with Sainted Hero’s Grave, which also contains a Black Knife Assassin but is a separate location with a different encounter and reward.

The most direct approach is from the Abandoned Coffin Site of Grace. From there, ride north-west across the shallow water. The entrance is on the lake’s eastern side, near the rocky bank. Another useful reference point is Erdtree-Gazing Hill, from which the cave lies to the north-west.

Included areas

Sage’s Cave is compact, but its internal structure has several distinct functional sections:

These are all part of Sage’s Cave. The surrounding lake, the Abandoned Coffin Site of Grace and nearby open-world enemies are not.

Named directory: places and facilities

The last two names are descriptive labels for the dungeon’s two boss chambers rather than separate map locations.

Access and recommended preparation

You can enter Sage’s Cave as soon as you reach the Altus Plateau. No key item, quest progression or boss victory is required to open the cave. The usual ways of reaching Altus Plateau are through the Grand Lift of Dectus or the alternative route through Liurnia and the Ruin-Strewn Precipice, but Sage’s Cave itself has no separate access requirement.

The cave is most comfortable once you can reliably handle ordinary skeleton enemies and bosses with moderate health. Its enemies are not especially numerous, but its layout rewards careful searching rather than simply following the main passage.

Enemies and environmental hazards

Skeletons

Skeletons are the ordinary enemies most associated with Sage’s Cave. They are easy to overlook in the dark and may appear inactive until approached. After being knocked down, they can begin to reassemble. Attack the remains again before the revival animation finishes, otherwise the skeleton returns to the fight.

The cave’s narrow passages make it easy to be surrounded. Pulling one enemy at a time with a bow, throwing knife or spell is safer than rushing into dark rooms.

Skeletal snails

Necromancer Garris is supported by skeletal snails during his boss fight. These creatures appear from above and add pressure while Garris attacks. They are not the primary objective, but ignoring them can make the arena increasingly difficult to manage. The boss encounter continues to produce them until Garris is defeated.

Darkness and concealed walls

Sage’s Cave uses darkness and dead-end passages to encourage players to test walls. Some apparent dead ends are genuine, while others conceal rooms or routes. If a chamber seems to stop abruptly, check the nearby walls rather than assuming the path is finished.

The most important illusory wall leads towards Necromancer Garris. Other concealed walls hide chests and equipment, making it possible to complete the cave’s main route while missing some of its best rewards.

Walkthrough

1. Activate the Site of Grace

Enter the cave and activate the Sage’s Cave Site of Grace. This is the dungeon’s only checkpoint. It is worth resting before exploring, since the first rooms contain skeletons and several concealed side paths.

Proceed through the opening passages, checking corners and walls as you go. The cave’s early rooms contain ordinary loot and crafting materials, but they also establish the dungeon’s main trick: several apparent dead ends are not actually dead ends.

2. Search the early chambers

Explore the first rooms carefully and collect the consumables and crafting materials before going deeper. You will encounter skeletons in cramped spaces, so avoid sprinting past them unless you are deliberately making a boss attempt.

Several chests are placed behind or near illusory walls. Strike suspicious walls or roll into them to reveal hidden rooms. The most useful early discoveries include Lost Ashes of War, Rejuvenating Boluses, Golden Rune [5], Silver-Pickled Fowl Foot, Black Hood and Candletree Wooden Shield.

3. Collect the equipment in the concealed rooms

Continue checking side chambers before committing to either boss. Sage’s Cave contains a particularly valuable group of equipment pieces:

The Raptor’s Black Feathers are especially useful for builds that rely on jump attacks, while the Raptor Talons provide a claw weapon with the same general aesthetic. The Skeletal Mask and Black Hood are separate headpieces, not parts of a single complete set.

4. Find the route to Necromancer Garris

One illusory wall opens a route to a separate room containing Necromancer Garris. This fight is optional and independent from the Black Knife Assassin encounter. If you reach the end of the cave and have not found Garris, return through the earlier chambers and test the walls around apparent dead ends.

Garris uses a flail-like weapon called Family Heads, a staff and death-themed magic. His signature weapon is made from three copper heads said to represent his wife and two children. During the fight, skeletal snails drop into the arena and can distract you while Garris casts or swings.

Tactics for Necromancer Garris

Defeating Garris awards Family Heads and runes. The boss’s lore is tied to ancient death magic and vengeful spirits, rather than to the Black Knife Assassins.

5. Reach the Black Knife Assassin

The Black Knife Assassin is found in a separate boss chamber. The assassin is invisible, with movement indicated by ripples in the water. The Sentry’s Torch makes her visible, which can be particularly helpful for players who struggle to judge the direction of the ripples.

The arena itself is relatively open, but the boss attacks quickly and can punish long recovery animations. The assassin can close distance rapidly and uses fast melee strings, so avoid attacking simply because she has disappeared from view. Watch the water disturbance and wait for a clear opening.

Tactics for the Black Knife Assassin

The Sage’s Cave version has its own encounter data and reward. Do not confuse it with the Black Knife Assassin in Sainted Hero’s Grave, which is a separate fight and drops the Black Knife instead. The Sage’s Cave assassin drops the Concealing Veil.

Black Knife Assassin

The Concealing Veil is a talisman associated with concealment and is the principal reward for this version of the Black Knife Assassin. The boss is not connected to the Sainted Hero’s Grave encounter for purposes of rewards or completion.

Necromancer Garris

Garris is a heretical sage who rediscovered an ancient death hex. His equipment and spell selection reinforce that theme: he wields Family Heads, uses a Prince of Death’s Staff and employs death-related magic.

Nearby connections

Sage’s Cave sits in the western Altus Plateau, in the same broad landscape as several other optional locations. These should be treated as neighbouring areas rather than subareas of the cave.

Sage’s Cave has no road, merchant, shop or resident NPC outside its boss encounters. The nearby roads and open-world enemies belong to the Altus Plateau rather than to the cave itself.

Lore and themes

The cave’s two bosses represent different aspects of death in Elden Ring. The Black Knife Assassin belongs to the wider group of assassins associated with the Night of Black Knives, while Necromancer Garris is an isolated practitioner of death magic. Their co-location appears to be thematic rather than the result of a stated alliance.

Garris’s weapon description gives the clearest local lore. Family Heads consists of three copper heads made in the likeness of his wife and two children, and its associated power calls forth vengeful spirits. His use of a Prince of Death’s Staff and death sorcery further identifies him as a practitioner concerned with the dead.

The cave’s concealed rooms and unusually generous equipment selection also give it the feel of a hidden storehouse or lair. That interpretation is atmospheric rather than confirmed in the game’s explicit text, so it should not be treated as established history.

---

Progression and missable content

Sage’s Cave is optional and does not block the main story. Its rewards remain available during a normal playthrough, and no known main-story progression state is required to unlock either boss.

The important practical miss is not a timed quest event, but failing to investigate the illusory walls. A player can reach one boss, collect the obvious loot and leave without discovering Garris or the concealed equipment. If you are completing the cave, treat every dead end as a possible secret until you have checked it.

The Black Knife Assassin is also easy to misidentify because the cave contains another Black Knife Assassin in the wider Altus Plateau region. The Sainted Hero’s Grave encounter is separate, has different statistics and drops the Black Knife rather than Concealing Veil.
