About this guide
Scope: Elden Ring base game, including the surrounding Murkwater River approach where it directly affects access to the cave. The nearby Bloody Finger Nerijus encounter is treated as an approach event rather than part of the cave’s interior.
Overview
Murkwater Cave is a small early-game dungeon in Limgrave, hidden in the shallow channel of the Murkwater River. Its modest size disguises its importance. The cave introduces one of Elden Ring’s most persistent recurring characters, Patches, provides an early merchant with several unusually useful tools, and sits beside a scripted invasion involving Bloody Finger Nerijus and Bloody Finger Hunter Yura.
Inside, the player deals with former foot soldiers who have become highwaymen. The dungeon’s main challenge is not its map size but its alarm traps, enclosed combat spaces and ambush-style layout. Once the cave’s boss encounter is resolved, it can become a useful early shopping stop, particularly before confronting Margit, the Fell Omen.
Murkwater Cave is optional. It is not required for access to Stormveil Castle, but it is worth visiting early because its rewards, merchant stock and character events have consequences beyond Limgrave.
Reaching the cave
The approach is more dangerous than the cave’s opening rooms because Nerijus uses Reduvia and builds up bleed quickly. Torrent cannot be used during the invasion, and the player cannot simply ride into the cave once the event begins. Retreating towards the bridge gives Yura time to enter the encounter, provided the relevant conditions are met.
Reaching the cave: steps
- 01Start from Agheel Lake North or another nearby Limgrave Site of Grace.
- 02Travel along the Murkwater River beneath the bridge.
- 03Continue towards the cave entrance.
- 04Be prepared for Bloody Finger Nerijus to invade before you can enter.
- 05Once the invasion is resolved or escaped, enter Murkwater Cave and activate its Site of Grace.
Nearby connections
The cave does not provide a shortcut to another dungeon or region. Its long-term significance comes from Patches and the connected NPC progression rather than from its physical layout.
---
Murkwater Cave is a natural cavern converted into a makeshift hideout. The opening passage leads to a broad, dimly lit chamber where the highwaymen camp around discarded belongings and stolen goods. The area is deliberately arranged to punish rushing.
Several tripwires are stretched across the floor. Stepping on one rings a bell and alerts nearby enemies. The wires are easy to overlook in the low light, particularly during a first visit. They do not create a complicated puzzle, but they make stealth and positioning useful.
The main chamber contains bushes and other cover that can be used to break enemy sightlines. A short route leads onwards to the fog gate for Patches’ room. The boss room is not a large arena. Patches uses the central chest and surrounding space to create a close-range ambush rather than a conventional grand boss battle.
Internal named areas
The last three are descriptive subareas rather than separately named map locations.

Nearby connections
| Name | Details |
|---|---|
| Agheel Lake North | The most convenient nearby starting point for reaching the cave. |
| Murkwater River | The direct geographical approach. |
| The bridge above the river | The area around the bridge is relevant to the Nerijus invasion and Yura’s assistance. |
| Stormhill and Stormveil Castle | Murkwater Cave is an optional side location encountered during the early Limgrave route towards Stormveil. |
| Mistwood | Later, Patches’ chest can teleport the player to the Mistwood as part of his optional follow-up event. |
Internal named areas
Highwaymen
The regular enemies inside Murkwater Cave are Highwaymen. They are former foot soldiers from a defeated army who have turned to banditry. Patches describes them as survivors abandoned by their former lord. Their equipment and behaviour support this history: they retain military-style clothing and weapons but now operate as roadside robbers.
Highwaymen may use short swords or daggers. Some can throw poison pots, making it dangerous to fight several in close quarters. Their attacks are quick but readable, and they are vulnerable to ordinary stagger, parries and backstabs.
The equipment and weapon drops are random enemy drops rather than guaranteed dungeon rewards. The Highwaymen are also an early farming option for the dagger and their cloth armour pieces.
Enemy directory
There is no separate named faction leader among the ordinary enemies. Patches is the bandit group’s leader and the cave’s principal named resident.
---
Bloody Finger Nerijus appears on the riverbed outside Murkwater Cave as the player approaches the entrance. He is a member of the Bloody Fingers and attacks with Reduvia, giving the encounter rapid bleed-oriented melee and ranged pressure.
This invasion is not technically an interior cave event, but it is closely associated with Murkwater Cave because it can prevent entry. Once it begins:
Defeating Nerijus rewards the Reduvia dagger. If the player is struggling, retreating towards the bridge rather than fighting at the cave mouth usually gives more room to manoeuvre and allows Yura’s sign to appear. Nerijus has relatively low poise, so charged heavy attacks, jump attacks and other stagger-heavy options are effective.
Exterior named directory
Yura is not a permanent resident of the cave and does not remain there after the encounter.

Triggering the fight
Enter the fog gate at the end of the cave and open the treasure chest in the room. The chest contains the Cloth Garb and Cloth Trousers. Opening it causes Patches to drop down and accuse the player of stealing his property.
Patches is armed with a spear and shield. His attacks are manageable but can be awkward in the confined room, especially if the player remains directly in front of him. He uses thrusting attacks, shield protection and occasional thrown consumables. The most important feature of the encounter is its surrender state.
When Patches’ health is reduced to roughly half, he stops fighting and asks for mercy. At this point the player can:
If the player keeps attacking after his surrender, he can become hostile again. The encounter provides the Grovel for Mercy gesture when he first yields, and a further continuation can involve Extreme Repentance if the player pushes the situation too far before using the gesture.
Triggering the fight: steps
- 01Stop attacking.
- 02Allow the encounter to end without killing him.
- 03Reload the area.
- 04Return to his room and speak to him.
- 05Access his shop.
Spare or kill Patches?
Killing him provides the following reported rewards in a standard New Game cycle:
The exact value of rune rewards can vary by game cycle and modifiers, but the equipment and Bell Bearing are the meaningful fixed outcomes.
For most players, sparing Patches is the better choice. His shop contains limited early-game items, his questline continues across several regions, and killing him is not necessary to obtain the cave’s most useful rewards.
Patches directory
---
After Patches is spared and the area is reloaded, he becomes a merchant in the cave. His shop is unusually useful for an early Limgrave visit because it combines ordinary supplies, multiplayer items, a Stonesword Key and Margit’s Shackle.
Spare or kill Patches?
| Choice | Immediate result | Long-term result |
|---|---|---|
| Spare Patches | He surrenders and later becomes a merchant | Patches’ wider questline remains available |
| Kill Patches | Receive his equipment, runes and Bell Bearing | His normal NPC progression is lost |
| Attack after surrender, then calm him | Can preserve the encounter if the correct gesture is used | Still allows the merchant state after reloading |
Available stock
Some stock limits increase after Patches moves to Scenic Isle in Liurnia. The table therefore describes the cave shop’s standard stock and notes where later progression can expand it.

| Festering Bloody Finger | 1,000 | 5 initially, later expanded |
| Stonesword Key | 5,000 | 1 |

Margit’s Shackle
Margit’s Shackle is the most important purchase for many early-game players. It is a reusable tool that briefly binds Margit, the Fell Omen, during the first phase of his fight. It can be used more than once, although it does not simply win the battle on its own.
The item also produces a large non-damaging effect that can interact with certain environmental objects, including some illusionary walls and trap mechanisms. Its main reason for being associated with Murkwater Cave, however, is its value against Margit. It costs 5,000 runes, so players who want it should avoid spending all their early runes before returning to Patches.
---
Fixed loot
The cave’s main fixed item reward is the chest in Patches’ room:
These are two pieces of the Guilty Set. The Guilty Hood is obtained elsewhere, so Murkwater Cave does not contain the complete set.
Materials
Some of these are environmental pickups, while others can come from enemy drops. Their presence should not be confused with unique dungeon rewards. The highwaymen may also drop bolts and their own weapons or armour pieces.

Recommended route
- 01Clear the river approach before committing to the cave. Deal with nearby enemies if they are likely to interfere with the Nerijus invasion.
- 02Approach the cave entrance carefully. Nerijus invades near the mouth.
- 03Retreat towards the bridge if necessary. This can give Yura time to join the fight.
- 04Defeat Nerijus and collect Reduvia.
- 05Enter Murkwater Cave and activate the Site of Grace.
- 06Move through the opening chamber slowly. Watch for tripwires.
- 07Pull highwaymen into manageable groups. Avoid triggering the alarm while surrounded.
- 08Collect the Cloth Garb and Cloth Trousers from the chest.
- 09Fight Patches until he reaches his surrender point.
- 10Stop attacking and reload the area.
- 11Return to Patches and inspect his shop.
- 12Consider buying Margit’s Shackle, the Stonesword Key or the Missionary’s Cookbook [2].
- 13Before leaving, check the cave again after later Patches progression.
Combat advice for the cave
The alarm traps are more dangerous as a positioning problem than as a damage source. If a tripwire is triggered, the highwaymen become alert and may surround the player. Use the room’s bushes and corners to break line of sight, and consider drawing one enemy out at a time.
Daggers and short swords make the highwaymen fast at close range, but their attacks are vulnerable to parries and backstabs. Shields are useful for absorbing their first flurry, while jump attacks and charged heavies can stagger them.
The boss room is cramped. Staying close to Patches allows him to use his spear and shield effectively, but moving around the outside of the room can create opportunities for backstabs. Do not use a large damaging attack carelessly once he begins begging for mercy.
---
The second chest
After Patches has become friendly, another visit to his room can reveal an additional chest. Opening it triggers a trap that transports the player to the Mistwood. Fast travel is disabled until the player rests at a Site of Grace.
This is an intentional Patches trick rather than a permanent dungeon shortcut. It is safe to use if the player is prepared to escape the destination area, but it can be inconvenient for a character carrying a large number of runes. Returning to Patches afterwards advances further dialogue and can award the Calm Down! gesture if it has not been obtained through another stage of his progression.
Later return to Murkwater Cave
Patches’ questline eventually brings him back to Murkwater Cave. After his later appearances in Liurnia, Mt Gelmir, Volcano Manor and the Shaded Castle, the player can return to the cave and trigger another encounter by opening the chest in his old room.
Do not kill him during this later encounter if you want the related gesture and final dialogue. He eventually recognises the player and gives the Patches’ Crouch gesture. The exact timing of his return depends on the player’s wider Patches progression, so the cave’s NPC state is not fixed for the entire game.
Missable or state-dependent content
The following can be lost or altered by progression choices:
---
Murkwater Cave is a small example of the social collapse visible throughout Limgrave. Its bandits are not presented as a formal army or organised criminal faction. They are displaced soldiers who have survived the defeat of their former lord and now prey upon travellers.
Patches’ dialogue identifies them as foot soldiers discarded after serving their lord. Their presence in a river cave suggests a practical refuge rather than a permanent settlement: they are close to a road and bridge, but hidden enough to ambush people travelling between Limgrave’s early routes.
Patches himself fits the cave perfectly. He is both the bandits’ leader and a parasite on the same roadside economy. He steals, sells, deceives and relocates whenever it benefits him. The cave is therefore less a home than one temporary branch of his travelling business.
His later appearances extend the same pattern across the Lands Between. Murkwater Cave is the first point where the player sees that Patches’ apparent surrender is not the end of an encounter but the beginning of a longer relationship built around mistrust, commerce and repeated betrayal.
---
Before leaving Murkwater Cave, check that you have:
---
Is Murkwater Cave required?
No. It is an optional Limgrave dungeon. Its main value is Patches, his shop, the cave’s armour pieces and the nearby Nerijus invasion.
Is Bloody Finger Nerijus inside the cave?
No. Nerijus invades on the riverbed outside the entrance. The encounter is directly connected to access but is geographically separate from the dungeon interior.
Should I kill Patches?
Usually not. Sparing him gives access to his shop and keeps his wider questline available. Killing him gives his equipment and Bell Bearing but removes those NPC services for the rest of the playthrough.
Where is Margit’s Shackle?
Patches sells one copy in Murkwater Cave for 5,000 runes after he becomes a merchant.
Can I get the complete Guilty Set here?
No. Murkwater Cave contains the Cloth Garb and Cloth Trousers. The Guilty Hood is found elsewhere.
Can Patches return to Murkwater Cave?
Yes. His later progression can bring him back to the cave for another encounter, provided he was not killed earlier. Spare him again to receive the later dialogue and Patches’ Crouch gesture.
